Initial Assessment (90 minutes): $175
Follow-up Visit (60 minutes): $150 
 
Policies:
Payment is requested at the time of the appointment and can be made with cash, credit card, or personal check. A $30 fee will be assessed for returned checks. Payment may also be made through PayPal.
Health Insurance is not accepted at this time, however a Superbill will be provided upon request. A prescription from your doctor will be required in order to provide a Superbill.
Phone consultations lasting over 15 minutes will be charged at 15- minute increments at the office rate.
Sliding scale rates are available upon request.
Please provide 24-hours notice when canceling appointments. Appointments cancelled with
less than 24-hours notice will be subject to a 50% charge.

Discounted rates are available for sessions purchased in advance: 5% off three visits, 10% off six visits, and 15% off 12 visits. Visits must be used within 1 year from date of purchase.
